Dr. Jekyll and Mr. Hyde for NES without a shadow of doubt. That game was so broken that it's impossible to complete... unless you, I dunno, specialize in playing broken games. ._.

As for hard games which aren't ridiculously broken, that would come down to Battletoads, Yars' Revenge and Centipede. No wait, I'll make that Millipede. The ever manly dropping nukes (Or something like that...) just add to the difficulty on a psychological level. 0_o
