It could be that she was shocked he looked like Scar. I always thought that she made that face because she was shocked that what she did left that much of a mark, and maybe she felt bad, I don't know.

I think she was shocked because it reminded her of Scar, who was also a murderer, (Zira says Nuka's death - Kovu's older brother - is his fault).

I just thought she was shocked and disappointed on Kovu. He looked a lot like Scar but didn't act like him (although he "killed" his big brother just like Scar had done - how ironic).

I have noticed Ziras reaction before, but it’s never struck me as fear.. more surprise or perhaps shock mixed with anger. I can’t really picture Zira being scared of anything, much less Kovu being similar to Scar.. She keeps claiming he’s just like him anyway.
